Packard Health's founders, Board of Trustees, and staff share a vision
of a healthy community in which every single member has a dependable
health care home with access to friendly, continuous, first-rate health
care. We maintain an unwavering dedication to providing the best care
possible for our patients and community by integrating primary health
care, mental health care, health promotion, and disease management programs.
Packard Health's doctors get to know their patients individually, and
patients are always treated with the same dignity and esteem that all
people deserve, regardless of their background or circumstances. Our
model of continuous care begins at birth and lasts through the end of
life. We work with our patients to help them stay healthy, in body and
in mind –not just to cure them in a crisis.
To minimize barriers to care, Packard Health staff members translate
into six different languages and the clinic offers evening and weekend
hours.
Additionally, Packard Health has established programs to assist our
community members with other health-related needs: we have a food pantry,
and medication assistance and insurance enrollment support programs
to help our patients fund and maintain their health and wellness.
Primary Health Care
Packard Health is a growing family practice that offers a full range
of primary health care services to both insured and uninsured members
of the Washtenaw County community. Patients choose a doctor and schedule
all of their care, from routine check-ups to urgent treatments, with
the same doctor. This ongoing relationship with a single doctor, as
well as with the general staff, is an important foundation for building
trust, encouraging communication, and forging an effective partnership.
Each physician works with his/her patients individually to plan their
care. This planning includes the care administered by Packard Health–both
primary and behavioral health care–as well as care from specialists,
hospitals, and other health care providers.
Mental Health Care
In collaboration with the Washtenaw County Health Organization and
others, Packard Health offers fully integrated psychiatric, psychological,
and counseling services.
Health Promotion and Chronic Disease Management
Packard Health's collaborations with the Washtenaw County Health Plan
and others have fostered the development of excellent health promotion
and chronic disease management programs. We believe it is critical to
educate patients about chronic diseases and to coach them in the improvement
of their nutritional, exercise, and other essential health behaviors.
