Behavioral Health Therapist

JOB SUMMARY:

The clinician shall provide diagnostic assessment and brief solution focused therapy using a variety of treatment modalities. The role provides Behavioral Health Services in an integrated primary care setting with FQHC designation.  Packard Health services the needs of the patients and community through a collaborative model, with behavioral health service clinicians who are skilled in establishing and maintaining effective professional and therapeutic relationships.

ESSENTIAL JOB DUTIES: 

Specific responsibilities may include, but are not limited to:

1. Consistently provide service excellence to all patients, family members, visitors, volunteers and coworkers
2. Must demonstrate working knowledge of DSM-5 diagnostic criteria
3. Complete Diagnostic Assessments; incorporate client participation in treatment plans
4. Provide documentation that demonstrates services are medically necessary
5. Work with multi-disciplinary team
6. Coordinate services with other community agencies
7. Initiate transition/discharge planning; provide appropriate services
8. Perform psychosocial evaluations and documentation and provide psychotherapeutic interventions that are appropriate to the client's age, education, cultural and ethnic background, medical status and functional ability
9. Document all patient related activity in a timely manner and according to agency policy and legal and ethical standards
10. Performs all administrative tasks related to his/her job assignments and other designated administrative tasks in timely manner.
11. Provide psychotherapy for patients, families and groups, and consult other professionals as needed
12. Participate in clinical utilization review and interdisciplinary team sessions as needed, for the purpose of assisting in patient assessment, development and revision of treatment goals, and discharge planning
13. In conjunction with the client and a multidisciplinary treatment team, complete treatment plans in a timely manner, continually evolving the treatment plan to reflect the changing clinical needs of the client and the treatment
14. Assist in planning and evaluation sessions with the organization which focus on program development, organization and innovation
15. Consult with Behavioral Health Specialist, medical director, and colleagues when clinical decisions regarding risk arise
16. Participate in seminars, workshops or other community education programs as required to maintain professional licensure. Participate in training sessions
17. Deliver care in accordance with appropriate standards of care and practice, including patient rights and protection of confidentiality (e.g. DPH, HIPAA)
18. Comply with program policies and procedures
19. Act as a resource to the staff where services are rendered. Support quality improvement activities
20. Demonstrate appropriate professional judgment and effective use of time
21. Act as a resource for clinical and procedural expertise to other staff members through mutual consultation
22. Work cooperatively with the Finance department to keep client accounts current
23. Provide timely responses to requests for credentialing and paneling documentation and work collaboratively with Packard Health in regards to eligibility.
24. Perform other work related duties as assigned/requested by supervisor

SUPERVISION RECEIVED: Reports to the Medical Director

EDUCATION/EXPERIENCE/TRAINING

• Licensed Masters prepared Social Worker
• Licensed in the state of Michigan
• Minimum 5 years of recent outpatient behavioral health experience preferred
• Substance abuse experience preferred
• Bilingual desired
