Chronic Disease Management

If you live with diabetes, high blood pressure, asthma, heart disease or other chronic conditions, Packard Health's team-based, personal approach can make a difference in your life.

Working together with an RN care manager and RN nutritionist, you'll learn important self-management skills and home monitoring, medication management, diet and exercise. We can assist with coordinating your care with specialists.

If you have diabetes, our Certified Diabetes Educator can help you set priorities and coach you on many strategies to make managing your diabetes easier.

Managing a chronic disease can improve your quality of life, prevent or minimize effects of a disease, and reduce your health care costs. We are here to help.
