Packard Health is a Federally Qualified Health Center (FQHC) serving our patients and community through an integrated, collaborative model within a primary care setting. Behavioral Health clinicians establish and maintain effective professional and therapeutic relationships with clients and provide diagnostic assessment and brief solution focused therapy using a variety of treatment modalities.

Essential functions

 Specific responsibilities may include, but are not limited to:

  • Consistently provide service excellence to all patients, family members, visitors, volunteers and coworkers
  • Must demonstrate working knowledge of DSM-5 diagnostic criteria
  • Complete Diagnostic Assessments; incorporate client participation in treatment plans
  • Provide documentation that demonstrates services are medically necessary
  • Work with multi-disciplinary team
  • Coordinate services with other community agencies
  • Initiate transition/discharge planning; provide appropriate services
  • Perform psychosocial evaluations and documentation and provide psychotherapeutic interventions that are appropriate to the client's age, education, cultural and ethnic background, medical status and functional ability
  • Document all patient related activity in a timely manner and according to agency policy and legal and ethical standards
  • Performs all administrative tasks related to his/her job assignments and other designated administrative tasks in timely manner
  • Provide psychotherapy for patients, families and groups, and consult other professionals as needed
  • Participate in clinical utilization review and interdisciplinary team sessions as needed, for the purpose of assisting in patient assessment, development and revision of treatment goals, and discharge planning
  • In conjunction with the client and a multidisciplinary treatment team, complete treatment plans in a timely manner, continually evolving the treatment plan to reflect the changing clinical needs of the client and the treatment
  • Assist in planning and evaluation sessions with the organization which focus on program development, organization and innovation
  • Consult with Behavioral Health Specialist, medical director, and colleagues when clinical decisions regarding risk arise
  • Participate in seminars, workshops or other community education programs as required to maintain professional licensure. Participate in training sessions
  • Deliver care in accordance with appropriate standards of care and practice, including patient rights and protection of confidentiality (e.g. DPH, HIPAA)
  • Comply with program policies and procedures
  • Act as a resource to the staff where services are rendered. Support quality improvement activities
  • Demonstrate appropriate professional judgment and effective use of time
  • Act as a resource for clinical and procedural expertise to other staff members through mutual consultation
  • Work cooperatively with the Finance department to keep client accounts current
  • Provide timely responses to requests for credentialing and paneling documentation and work collaboratively with Packard Health in regards to eligibility.
  • Perform other work related duties as assigned/requested by supervisor

  • Must be able to lift 15 pounds at times

Required education and experience

  • Licensed Masters prepared Social Worker or Licensed Professional Counselor
  • Licensed in the state of Michigan
  • Recent outpatient behavioral health experience preferred
  • Substance abuse experience preferred
  • Bilingual desired
  • At least 2 years related experience
