										<content:encoded><![CDATA[<p>Packard Health was recently awarded a total of $81,541 in Quality Improvement Awards (QIA) from the U.S. Department of Health and Human Services’ <a href="https://bphc.hrsa.gov/programopportunities/fundingopportunities/qualityimprovement/states/MI.html">Health Resources and Services Administration (HRSA) Health Center Program</a>.</p>



<p>As a Federally Qualified Health Center, Packard Health submits data reflecting its services to the HRSA annually. Awards are given in recognition of high performance or significant quality improvement gains and are to be used to further strengthen quality improvement activities.</p>



<p>The categories that Packard Health received awards in and their corresponding award amounts include:</p>



<ul class="wp-block-list"><li>Clinical Quality Improvers (demonstrating improvement in quality measures) &#8211; $24,416</li><li>Access Enhancers (providing more services to more patients) &#8211; $5,750</li><li>Health Disparities Reducers (made at least a 10% improvement across different racial/ethnic groups) &#8211; $43,125</li><li>Advancing Health Information Technology for Quality (collecting social determinants of health data and using it to drive care) &#8211; $8,250</li></ul>

<p>Lori received a great honor last week &#8211; 2019 Hometown Health Hero from the Michigan Public Health Week Partnership. Lori created a program called <strong>F-SHARP</strong> (Family Shelter Assessment and Referral Program). She visits families in shelters across the county and connects them with health care at Packard and The Corner Health Center.</p>
<p>On her visits to INH-Alpha House, SafeHouse Center, Ozone House and Staples Family Center Lori meets with families, identifying immediate health needs, making referrals, and setting up appointments to see her at <strong>Packard Health</strong> or <strong>The Corner Health Center</strong>.</p>

<p>The program is funded by <strong>Washtenaw Coordinated Funders.&nbsp;</strong></p>
<p>Housing instability and inability to access health care go hand-in-hand. Lori serves the mothers, fathers and children who deal with those stressors every day. Her specialty is building trust.</p>
<p>We applaud Lori for her &#8220;get-out-there-and-do-it&#8221; attitude.&nbsp; She saw a need; she created a solution.</p>

<p>Packard Health is honored to receive a grant from Blue Cross Blue Shield of Michigan, the Blue Cross Blue Shield of Michigan Foundation, the Ethel and James Flinn Foundation and the Michigan Department of Health and Human Services.</p>
<p>This grant of $70,820 will support a behavioral therapist at our Ypsilanti office, improving access to integrated health care services for low-income, uninsured patients managing mental illness, behavioral health problems or substance abuse disorders.</p>
<p>On hand for the award ceremony on March 31 were <strong>Rebekah Warren</strong>, State Senator, D-District 18 and member of Michigan Mental Health Commission, <strong>Raymond Rion, MD</strong>, Packard Health Executive Director, and <strong>Nora Maloy, DrPH</strong>, Director of Programs, Blue Cross Blue Shield of Michigan Foundation, and Packard Health board member.</p>
