Log in to your patient portal to communicate with your health care team, view test results and more.<\/p>\n\n\n\n<p class=\"has-paragraph-large-font-size wp-block-paragraph\"><strong>If you have a MyChart username and password, please log in using the box on this page. If not, follow the instructions below to sign up.<\/strong><\/p>\n\n\n\n<details class=\"wp-block-details is-layout-flow wp-block-details-is-layout-flow\"><summary>Sign Up For MyChart<\/summary>\n<p class=\"wp-block-paragraph\">If you do NOT have a username and password, follow these instructions to sign up for your MyChart patient portal:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>DO NOT click the \u201cSign up\u201d button in the login box, instead start the process by emailing <a href=\"mailto:MyChart_Help@packardhealth.org\">MyChart_Help@packardhealth.org<\/a> with the following information:\n<ul class=\"wp-block-list\">\n<li><strong>Your full name<\/strong><\/li>\n\n\n\n<li><strong>Date of birth<\/strong><\/li>\n\n\n\n<li><strong>Phone number<\/strong><\/li>\n\n\n\n<li><strong>Whether you want to activate via email or text<\/strong><\/li>\n\n\n\n<li><strong>The name of your Packard Health primary care provider<\/strong><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Once we receive your email our staff will activate your account as soon as possible. You will receive a text or email message within 3\u20135 business days with instructions to complete your activation.<\/li>\n\n\n\n<li>You may also activate your MyChart portal at your next office visit.<\/li>\n\n\n\n<li>If you urgently need immediate access please call us at <a href=\"\/tel: 7349711073\">(734) 971-1073<\/a>.<\/li>\n<\/ul>\n<\/details>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>What can you do in MyChart?<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Communicate with your doctor.<\/strong> Get answers to your medical questions from the comfort of your own home. <em>Please allow up to 72 hours for a response to MyChart messages. Sending multiple messages may slow down our ability to reply.<\/em><\/li>\n\n\n\n<li><strong>Access your test results.<\/strong> No more waiting for a phone call or letter \u2014 view your results and your doctor&#8217;s comments within days.<\/li>\n\n\n\n<li><strong>Request prescription refills. <\/strong>Send a refill request for any of your refillable medications. <em>Prescription refill requests may take up to 7 business days.<\/em><\/li>\n\n\n\n<li><strong>Manage your appointments.<\/strong> Schedule your next appointment, or view details of your past and upcoming appointments. <\/li>\n\n\n\n<li><strong>View your billing account.<\/strong> See account detail, view statements, sign up for paperless billing and pay your bill.<\/li>\n\n\n\n<li><strong>Complete eCheck-in before your visit.<\/strong> Skip the paperwork at the office by completing key steps ahead of time, including signing documents, updating your health history, uploading insurance cards, making a copay, and sharing questions for your provider.<\/li>\n\n\n\n<li><strong>MyChart Support<\/strong> <a href=\"mailto:MyChart_Help@packardhealth.org\">Email MyChart_Help@packardhealth.org<\/a> to submit a MyChart support request.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\" style=\"font-size:14px\">Note: Packard Health&#8217;s Epic software is hosted by University of Michigan Health-West. Packard Health remains a separate entity, and is not a part of UM Health-West or Michigan Medicine.
