MyChart is the best way to manage your health care. Login to your patient portal to communicate with your health care team, view test results and more!

Sign up for your MyChart patient portal!

Do NOT click the button in the window on the left, instead start the process by emailing MyChart_Help@packardhealth.org with the following information:

• Your full name,
• Whether you want to activate via email or text,
• The name of your Packard Health primary care provider.

Once we receive your email our staff will activate your account as soon as possible. You will receive a text or email message within 3 - 5 business days with instructions to complete your activation.

You may also activate your MyChart portal at your next office visit.
