Packard Health offers a uniquely integrated model of health care that combines primary care, mental health care, health promotion, and disease management programs in one comprehensive family practice setting. We establish ongoing relationships with our patients to help each individual achieve his/her fullest health potential, not just to treat an illness or ailment. Mental and Behavioral Health Services

Packard Health patients have access to psychiatry and social services for the treatment of mild to severe mental illness. Patients work together with medical and behavioral health care staff to plan and manage their medication and solution-oriented therapy. Co-sponsored by the Washtenaw Community Health Organization, the mental health program is part of Packard Health's nationally recognized integrated care approach.

Chronic Disease Management

Packard Health offers one-on-one and group education and social support for patients with asthma, chronic pain, diabetes, depression, bipolar disorder, heart disease, high cholesterol, and weight problems. Packard's registered dietician and certified diabetes educator collaborates with providers and patients to establish effective self-management practices for chronic conditions.

Health Promotion

Packard Health provides a full roster of health promotion activities to serve our patients. Programs are updated on the Health Promotion calendar.

Highlights include the summer Packard Pacers walking club (video), periodic smoking cessation classes, and HIV/AIDS education and testing.

In an effort to promote health throughout our community, Packard Health offers selected screenings and information at health fairs and other community events throughout the year. Packard providers are also available to speak on various health topics to organizations throughout the community. Packard helps train residents of Ann Arbor's public housing communities as Community Health Workers, volunteers who educate their neighbors about health and social services.

Insurance Enrollment

Patients can receive assistance with enrollment in publicly funded health plans such as Medicaid, MIChild, and the Washtenaw Health Plan through Packard's insurance enrollment service. Our patient advocates are available by appointment to review eligibility for public health plan enrollment.

Food Pantry and Medication Assistance

Packard Health's food pantry and medication assistance programs aid patients who are unable to meet their nutrition needs or afford prescription medication. The food pantry also offers nutrition education and food stability planning services. Through the medication assistance program, Packard Health provides medication samples and helps enroll patients in pharmaceutical company assistance programs.
