Healthy Michigan Medicaid Patients! Your Medical Coverage May Be at Risk - Act Now

Healthy Michigan Plan patients, please take note. You are asked to do a Health Risk Assessment with your primary care provider and pick a healthy behavior every year.
If you have NOT picked a healthy behavior in the last 12 months your medical coverage may change and you may lose some of your current benefits. Your health plan may change from a Healthy Michigan Plan to a MI Marketplace Option health plan. You may have fewer benefits and may lose dental and vision coverage.
There are two ways to get Health Risk Assessments done:
1. Call the Health Risk Assessment (HRA) Unit as soon as possible to complete a Healthy Michigan Plan HRA and pick a healthy behavior. 1-833-420-8278 Monday through Friday, 8:00 AM - 5:00 PM.
2. Call Packard Health 734-971-1073 and make an appointment with your provider. Complete form in the clinic and have it faxed.
